Almost all peripheral rheumatoid arthritis affects the hand and wrist joints. These joints are the first to involve rheumatoid arthritis and are the sites of late deformity. Early joint space narrowing, localized osteoporosis, and bone destruction of metacarpophalangeal and wrist joints. Late X-ray showed more obvious bone destruction, loss of joint space, joint fibrosis, bony ankylosis, joint dislocation or deformity. Therefore, it is very important to cure the disease of the hand and wrist joint early. The wrist is a very important part of the human body. Almost all patients with rheumatoid arthritis involve the hand and wrist joints. There are also hand and wrist joints alone or first. The typical early feature is the fusiform appearance of the proximal interphalangeal joint due to swelling, often accompanied by metacarpal fingers The joint is swollen symmetrically, and the distal interphalangeal joint is rarely involved. Soft tissue looseness and weakness can produce ulnar deflection of the fingers, often accompanied by volar subluxation of the proximal phalanx; ulnar deflection of the metacarpophalangeal joint often merges with the radial deflection of the radiometacarpal joint, resulting in a zigzag deformation of the hand . In advanced patients, "gooseneck" deformity and "button flower" deformity may occur. These changes will result in loss of hand strength. Painless swelling of the ulnar styloid process is one of its early signs. Volar synovial thickening and tenosynovitis can compress the median nerve under the transverse carpal ligament, causing "carpal tunnel syndrome". The palmar side of the thumb, index finger, and middle finger appear. The radial skin of the ring finger is abnormal and dull, and may also be accompanied by large Atrophy of thenar muscles. In the late stage, due to fibrous rigidity or bony rigidity, the wrist becomes unable to move, and the involvement of the distal radioulnar joint often severely impairs the pronation and supination movement. Ulnar head syndrome (including symptoms such as pain, restricted movement, and protrusion of the dorsal end of the ulna) can be seen in rheumatoid arthritis.
